Study on the correlation between serum NGAL and PCT levels and renal function outcome in patientswith sepsis-related acute kidney injury

Abstract
Objective To study the correlation between serum levels of neutrophil gelatinase-associated lipocalin(NGAL)and procalcitonin(PCT)and renal function outcome in patients with sepsis-related acute kidney injury(AKI).Methods A total of 62 cases of patients with sepsis complicated with AKI admitted to our hospital from September 2015 to September 2018 were selected,and divided into complete renal function recovery group with 19 cases,partial renal function recovery with 34 cases and non-recovery group with 9 cases,according to renal function 28 days after admission.The clinical data,serum NGAL and PCT levels in the three groups were compared.The influencing factors of renal function outcome were analyzed with the method of logistic regression.ROC curve was used to analyze the predictive value of serum NGAL and PCT levels for renal dysfunction.Results There were no statistically significant differences in sex,age,BMI,hypertension,diabetes mellitus,body temperature,respiratory rate,heart rate,systolic pressure,diastolic pressure,sepsis infection site,white blood cell count,hemoglobin content and albumin content,among the three groups(P>0.05);there were statistically significant differences in the number of failed organs,mechanical ventilation status,the use of anti-hypertension drugs,platelet count,APACHE II score,SOFA score,serum NGAL and PCT levels immediate enrollment and 6 consecutive days after enrollment between the two groups(P<0.05).Serum NGAL and PCT levels increased at enrollment,the number of concurrently failed organ>3,mechanical ventilation,APACHE II Score >30 were risk factors for poor outcome of renal function.The best cutoff points of NGAL and PCT to predict renal dysfunction were 99.17 and 5.4 respectively,the sensitivity values were 88.89% and 77.78%,and the specificity values were 81.48% and 79.63%,respectively.Conclusions The increase of serum NGAL and PCT levels is related to the poor prognosis of renal function in sepsis patients with AKI.Meanwhile,the number of concurrently failed organs,mechanical ventilation and APACHE II score also affect the prognosis of renal function.
