    A comparative study on the effectiveness of central venous catheter blockage treated with different concentration of urokinase in hemodialysis patient

    • Objective To investigate the effect of different doses of urokinase in hemodialysis patients on the blockage of central venous catheters.Methods Patients who received hemodialysis and CRRT were treated with central venous catheters from February 2015 to August 2017 in our hospital blood purification center. The patients were randomly divided into two groups:A (20,000 U/ml urokinase) and B (40,000 U/ml urokinase) and there were 15 patients in each group. Before the blood purification, patients with catheter obstruction, were given two doses of urokinase into the arterial end and venous end of the catheter respectively according to the catheter scale. Within the catheter, urokinase was used for thrombolysis for 30 minutes and then discarded. Then, blood flow, catheterization, complications and prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen content (FIB) and other parameters were recorded during hemodialysis.Results The rate of recanalization of catheter in group A was 60% (9/15) and blood flow was 195.0±14.6; at the same time, the rate of recanalization of catheter in group B was 93.3% (14/15), and blood flow was 227.0±12.5. There was significant difference between the two groups(P<0.05). There was no significant difference in the incidence of complications between the two groups,including PT, APTT and FIB between the two groups (P>0.05).Conclusions In group B, 40,000 U/ml urokinase treatment of catheter blockage is more effective, and this method is simple to operate, with a high success rate, the advantages of good security.
