    Effect of allopurinol on albuminuria in type 2 diabetic kidney disease with hyperuricemia

    • Objective To evaluate the therapeutic effect of allopurinol on albuminuria in patients with?type 2 diabetic kidney disease combined with hyperuricemia.Methods Sixty-one patients with type 2 diabetic kidney disease combined with hyperuricemia were randomly divided into two groups: research group (n=31) and control group (n=30). Patients in research group received routine treatment and low dose of allopurinol for 6 months, and those in control group were only treated with routine treatment. The fasting blood glucose (FBG), postprandial two-hour blood glucose (2hPBG), hemoglobin A1c (HbA1c), serum uric acid (SUA), blood urea nitrogen (BUN), serum creatinine (SCr), triacylglycerol (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), blood high-sensitivity creactive protein (hs-CRP), interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), albumin-to-creatinine ratio (ACR), urine albumin excretion (MAU), fasting insulin (FINS) and insulin resistance index (HOMA-IR) were evaluated pre-and post-treatment.Results Before the treatment, there were no significant differences in the general information and biochemical data between the two groups (P>0.05). Six months later, the levels of SUA, hs-CRP, IL-6, TNF-α, MAU, ACR, FBG, FINS and HOMA-IR in the research group were significantly decreased as compared with those in the control group (P<0.001).Conclusions Allopurinol is an effective in the treatment of albuminuria in patients with type 2 diabetes mellitus combined with hyperuricemia. It can protect renal function in early stage through controlling SUA and reducing insulin resistance to reduce urinary protein excretion.
