新型冠状病毒肺炎引起急性肾损伤的发病机制研究进展

摘要: 严重急性呼吸综合征冠状病毒2（severe acute respiratory syndrome coronavirus 2，SARS-CoV-2）是一种传染力极强的呼吸道病毒，越来越多的临床报道新型冠状病毒肺炎（coronavirus disease 2019，COVID-19）相关的急性肾损伤（acute kidney injury，AKI）。有研究表明，肾脏可能是SARS-CoV-2主要受累的靶器官，通过多因素引起肾脏功能的改变，包括病毒的直接毒力作用、肾素-血管紧张素-醛固酮系统的失衡、免疫系统失调、细胞因子风暴、横纹肌溶解、凝血功能障碍、脓毒血症、肾毒性药物、器官之间的串扰以及典型症状引起的肾前性因素。本文将COVID-19引起AKI的可能发病机制做一综述，为制定管理策略和寻找可靠的治疗方法提供依据。Abstract: Severe acute respiratory syndrome coronavirus 2（SARS-CoV-2）is a highly contagious respiratory virus. Acute kidney injury（AKI） associated with COVID-19（coronavirus disease 2019）has been reported in a growing number of clinical cases. Some recent studies noted that kidney might be a major target organ of SARS-COV-2. Multiple factors cause the changes of renal function， including direct viral toxicity，renin-angiotensin aldosterone system imbalances，disorders of immune system，cytokine storm，rhabdomyolysis，blood coagulation dysfunction，sepsis，drug-related renal toxicity and crosstalking between organ and symptoms caused by prerenal factors. This review summarized the potential pathogenesis of COVID-19-induced AKI.
