稀释风油精含漱液对血液透析口渴患者的干预效果研究

    Interventional effect of diluted essential balm mouthwash on hemodialysis patients with thirst

    • 摘要: 目的 探讨稀释风油精含漱液对血液透析患者口渴的干预效果研究。方法 选取2023年3月至5月在太和县人民医院(皖南医学院附属太和医院)血液净化中心行维持性血液透析存在口渴的43例患者为研究对象,采用随机数字表法分为A组(21例)和B组(22例),A组和B组分前后两个阶段交叉设计,每个阶段持续时间为2周,第一阶段完成后,经过2周洗脱期,将两组干预方法对调,进入第二阶段,步骤与方法同第一阶段。在第一阶段、第二阶段开始前后使用口渴视觉模拟自评量表(visual analogue scale,VAS)评估患者的口渴程度,口渴痛苦量表(the thirst distress scale,TDS )评估患者口渴痛苦程度,并计算透析间期体重增加值(interdialytic weight gain,IDWG)进行统计学处理。结果 第一阶段结束,A组患者口渴VAS得分(3.13 ± 2.97)分、TDS得分(10.14 ± 6.76)分、IDWG(2.51 ± 1.87) kg值均低于B组,差异均有统计学意义(P<0.05)。历经2周的洗脱期后,在第二阶段A组患者口渴VAS得分(6.84 ± 2.15)分、TDS得分(22.32 ± 4.75)分、IDWG值(3.12 ± 2.47) kg均高于B组,差异均有统计学意义(P<0.05)。结论 本研究结果表明把可内服风油精稀释后用于口腔含漱可以缓解血液透析患者口渴感,减轻因口渴带来的痛苦程度,减少IDWG,本措施简单可行,价格低廉安全性高,可纳入患者的日常生活习惯中。

       

      Abstract: Objective To explore the intervention effect of diluted essential balm mouthwash on thirst in hemodialysis patients. Methods A total of 43 patients with thirst who underwent maintenance hemodialysis at the Blood Purification Center of Taihe County People's Hospital (The Taihe Hospital of Wannan Medical College) from March 2023 to May 2023 were recruited. They were randomly divided into Group A (21 cases) and Group B (22 cases) using a random number table method. Group A and Group B were divided into two stages with a cross design, each stage lasting for 2 weeks. After the completion of the first stage, a 2-week washout period was passed, and the intervention methods of the two groups were switched to enter the second stage, with the same steps and methods as the first stage. Before and after the start of the first and second stages, the degree of thirst and thirst pain were assessed by the Visual Analogue Scale (VAS) and the Thirst Distress Scale (TDS), respectively. The Interdialytic Weight Gain (IDWG) was calculated for statistical processing. Results At the end of the first stage, the VAS score(3.13 ± 2.97), TDS score(10.14 ± 6.76), and average IDWG(2.51 ± 1.87) kg score of thirst in Group A were significantly lower than those of Group B (P<0.05). After a 2-week washout period, in the second stage, the VAS score(6.84 ± 2.15), TDS score(22.32 ± 4.75), and average IDWG(3.12 ± 2.47) kg score of group A were significantly higher than those of group B (P<0.05). Conclusion Diluting orally available essential balm for oral gargling can alleviate thirst and thirst-induced pain in hemodialysis patients, and reduce IDWG. This measure is simple, feasible, cost-effective, and safe, and can be included in the daily habits of patients.
