循环长链非编码RNA人类白细胞抗原复合体18、微小RNA-185-5p水平与糖尿病肾脏疾病患者血管钙化的相关性分析

    Correlation analysis of circulating LncRNA HCG18,miR-185-5p levels and vascular calcification in patients with diabetic kidney disease

    • 摘要: 目的 分析血清循环长链非编码RNA人类白细胞抗原复合体18(long non-coding RNA human leucocyte antigen complex group 18,LncRNA HCG18)、微小RNA-185-5p(microRNA-185-5p,miR-185-5p)水平与糖尿病肾脏疾病(diabetic kidney disease,DKD)患者血管钙化的相关性。方法 选取2018年10月至2021年9月于上海健康医学院附属崇明医院就诊的DKD患者145例,男性73例,女性72例,依据患者血管钙化情况分为钙化组70例,其中男性38例,女性32例,年龄范围46~78岁,年龄(57.14±14.27)岁;未钙化组75例,其中男性35例,女性40例,年龄范围44~77岁,年龄(59.47±15.06)岁。另选同期健康体检者70名为对照组。荧光定量PCR法检测血清LncRNA HCG18、miR-185-5p水平;采用全自动生化分析仪测定血钙、血磷、血肌酐(serum creatinine,Scr)水平;采用酶标仪测定碱性磷酸酶(alkaline phosphatase,ALP)、超敏C反应蛋白(high-sensitivity c-reactive protein,hs-CRP)及甲状旁腺激素(parathyroid hormone,PTH)水平;Pearson法分析DKD合并血管钙化患者血清LncRNA HCG18、miR-185-5p水平与血管钙化相关指标的相关性及血清LncRNA HCG18与miR-185-5p的相关性;受试者工作特征曲线分析血清LncRNA HCG18、miR-185-5p水平对DKD合并血管钙化的诊断价值。结果 钙化组、未钙化组血清LncRNA HCG18、血钙、血磷、Scr、ALP、hs-CRP及PTH均高于对照组(P<0.05),血清miR-185-5p水平低于对照组(P<0.05);钙化组血清LncRNA HCG18、血钙、血磷、Scr、ALP、hs-CRP及PTH均高于未钙化组(P<0.05),血清miR-185-5p水平低于未钙化组(P<0.05)。DKD合并血管钙化患者血清LncRNA HCG18与miR-185-5p呈负相关(P<0.05),血清LncRNA HCG18与血钙、血磷、Scr、ALP、hs-CRP及PTH均呈正相关(P<0.05),血清miR-185-5p与血钙、血磷、Scr、ALP、hs-CRP及PTH均呈负相关(P<0.05)。血清LncRNA HCG18、miR-185-5p水平及二者联合诊断DKD合并血管钙化的曲线下面积分别为0.820、0.758、0.880。结论 DKD血管钙化患者血清LncRNA HCG18呈高表达、miR-185-5p呈低表达,二者均对DKD患者血管钙化有一定的诊断价值,联合检测诊断价值更高。

       

      Abstract: Objective To explore the correlation between serum circulating long non-coding RNA human leukocyte antigen complex 18(LncRNA HCG18),microRNA-185-5p(miR-185-5p) levels and vascular calcification in patients with diabetic kidney disease(DKD).Methods From October 2018 to September 2021,145 DKD patients were recruited.According to the status of vascular calcification,they were divided into two groups of calcification(n=70) and non-calcification(n=75).And another 70 cases of normal physical examination during the same period were selected as control group.Fluorescent quantitative polymerase chain reaction(PCR) was utilized for detecting serum levels of LncRNA HCG18 and miR-185-5p;automatic biochemical analyzer was utilized for measuring the blood parameters of calcium(Ca2+),phosphorus(P3+) and creatinine(Scr);a microplate reader for measuring the levels of alkaline phosphatase(ALP),high-sensitivity C-reactive protein(hs-CRP) and parathyroid hormone(PTH);Pearson’s method for examining the correlation between serum LncRNA HCG18,miR-185-5p and vascular calcification-related parameters;receiver operating characteristic(ROC) curve plotted for examining the diagnostic value of serum LncRNA HCG18 and miR-185-5p levels for DKD with vascular calcification.Results Serum LncRNA HCG18,Ca2+,P3+,Scr,ALP,hs-CRP and PTH were higher in calcification and non-calcification groups than those in control group(P<0.05) and serum miR-185-5p level was lower than that in control group(P<0.05);serum LncRNA HCG18,Ca2+,P3+,Scr,ALP,hs-CRP and PTH were higher in calcification group than those in non-calcification group(P<0.05) while serum miR-185-5p level was lower than that in non-calcification group(P<0.05).Serum LncRNA HCG18 in DKD patients plus vascular calcification was negatively correlated with miR-185-5p(P<0.05),serum LncRNA HCG18 was positively correlated with Ca2+,P3+,Scr,ALP,hs-CRP and PTH(P<0.05),and miR-185-5p was negatively correlated with Ca2+,P3+,Scr,ALP,hs-CRP and PTH(P<0.05).The area under the curve of serum levels of LncRNA HCG18,miR-185-5p and the combination in the diagnosis of DKD with vascular calcification was 0.820,0.758 and 0.880 respectively.Conclusion Serum LncRNA HCG18 is highly expressed while miR-185-5p is lowly expressed in sera of DKD patients with vascular calcification.Both have some diagnostic value for vascular calcification in DKD patients and co-detection has a higher diagnostic value.
