骨矿物质代谢紊乱与慢性肾脏病免疫功能障碍及感染的相关性研究进展

    Research advances of correlations between bone and mineral metabolism disorders and immune dysfunction and infection in chronic kidney disease

    • 摘要: 慢性肾脏病(chronic kidney disease,CKD)患者存在免疫功能受损,易引起该人群患感染。CKD患者免疫功能受损的潜在机制是多因素的,研究表明矿物质和骨代谢异常(mineral andbone disorder,MBD)也参与了这一过程。本文系统性总结了MBD的生物标记物(维生素D、成纤维细胞生长因子23、甲状旁腺激素、钙、磷)在调节人体免疫功能方面的机制及其代谢紊乱与CKD患者感染风险的关系,其中重点介绍了维生素D在调节免疫及减少CKD患者感染风险方面的潜在价值,为临床降低CKD患者感染风险相关预防及治疗措施提供新的思路。

       

      Abstract: Chronic kidney disease(CKD) impairs immune function and renders patients susceptible to infection. The underlying mechanism of impaired immune function is multi-factorial. And mineral and bone disorder(MBD) is also involved. This review systematically summarized the mechanism of MBD biomarkers(vitamin D, fibroblast growth factor 23, parathyroid hormone, calcium & phosphorus) in regulating human immune function and the relationship between metabolic disorders and risk of infection. The potential values of vitamin D in regulating immunity and lowering the risk of infection provided rationales for clinically managing the risk of infection in CKD patients.
