    慢性肾脏病患者蛋白质能量消耗机制的研究进展

    Research advances for mechanism of protein-energy wasting in patients with chronic kidney disease

    • 摘要: 慢性肾脏病(chronic kidney disease,CKD)的发病率逐年上升,进展至终末期常并发多种代谢紊乱。蛋白质能量消耗(protein-energy wasting,PEW)是CKD患者常见的并发症,尤其是老年患者和接受血液透析治疗的CKD患者,由于脂肪和肌肉的丢失,肾功能衰竭速度加快,并发症和死亡风险明显增加。PEW发病机制复杂,核心环节涉及蛋白质合成与分解代谢失衡,除了与食欲下降、蛋白质及能量摄入不足以外,胰岛素抵抗、炎症、内分泌激素及代谢性酸中毒等所导致的高代谢状态均可促进蛋白质降解。另外,运动减少、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)也可导致蛋白质合成减少,进一步导致PEW,严重影响患者生活质量及远期预后。因此,及早干预并改善CKD患者的营养不良状态,具有非常重要的临床意义。

       

      Abstract: The incidence of chronic kidney disease(CKD) is rising yearly. It progresses to an end stage often complicated by a variety of metabolic disorders. Protein energy wasting(PEW) is a common complication of CKD patients, especially elders and CKD patients on hemodialysis. Due to a loss of fat and muscle, renal failure accelerates and there are greater risks of complications and mortality. The pathogenesis of PEW is so complex so that core link involves protein synthesis and catabolism imbalance. Besides a lower appetite, insufficient protein/energy intake, high metabolic states caused by insulin resistance, inflammation, endocrine hormones and metabolic acidosis all contribute to protein degradation. In addition, reduced exercise and insulin-like growth factor-1(IGF-1) can also lead to a decline in protein synthesis and PEW seriously affects patient's quality-of-life and long-term prognosis. Therefore early intervention and improvement of malnutrition status are vital for CKD patients.
